ANNEX 1 - MANDATORY CONDITIONS
Mandatory conditions where licence authorises supply of alcohol
(1) No supply of alcohol may be made under the premises licence -
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
(2) Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ADDITIONAL MANDATORY CONDITIONS.
The following mandatory conditions are to be applied to the licence stated above in accordance with section 3 (1) of The Licensing Act 2003 (Mandatory Licensing Conditions) Order 2010.
The following conditions shall come into force on 1st October 2010
4. (1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Conditions added following sub-committee hearing on 15th January 2014
1) CCTV be maintained at the premises and that any recordings be kept for no less than 3 months and be disclosed to any responsible authority on request
2) All staff employed at the premises be trained on the CCTV system
3) 1 members of staff as well as the personal licence holder be trained under the BIIAB course as soon as possible and within 6 months with refresher training within a reasonable period at regular intervals
4) Incident and refusal books be kept and maintained at the premises and be disclosed / be open to inspection upon reasonable request
It is recommended that such incident / refusal books be obtained from trading standards
